Health Requirements

Proof of Immunizations

All students are required to show proof of immunizations and adhere to the most current requirements regardless of age.

Students in K – 8 must have the following immunizations one week prior to the first day of school.
  • Polio
  • DTP
  • MMR
  • Hepatitis B
  • Varicella
*7th and 8th Grade Students must have documented proof of a Pertussis (Whooping Cough) Booster Immunization (Tdap) given after the age of 7. This is a CALIFORNIA STATE requirement and must be met with documentation turned in one week prior to the first day of the school year.

Report of Health Examination

All new students and current students moving into Grade 1 are required to have a completed Report of Health Examination for School Entry (PM171A) signed by their physician. This health examination must have taken place within 18 months of your student’s admission. The form is enclosed in the new student registration packet. Extra copies are available in the Nurse’s Office and by clicking on the link to the right.

Life-Threatening Allergies

Any child with a life threatening allergy must have an “ALLERGY ACTION PLAN” filled out and signed by a physician prior to the first day of school. If a child requires administration of an EPI-PEN for an allergic reaction, the EPI-PEN must be brought to the nurse’s office prior to the child’s first day of school.
